Our experienced exercise physiologists, specialized nurses, and licensed dietitians are dedicated to helping every patient achieve their heart-healthy objectives. Compared with those who don’t, patients who complete the cardiac rehabilitation program have a lower risk of hospital readmission, heart attack and even death.
What commitment does this program of care require?
After a referral from your doctor, a nurse conducts an admission interview. The nurse will help you identify your goals for the program. An exercise prescription, based on periodic exercise testing, is used to guide your physical activity. In addition, counseling is offered for risk factor reduction which includes: weight reduction, smoking cessation, management of diet and stress, and proper exercise techniques.
Cardiac Rehabilitation Program
Patients improve heart health through personalized exercise routines, one-on-one sessions with a dietitian, stress management, and telemetry and blood pressure monitoring. The supportive, welcoming environment motivates patients in group and individual sessions. Progress is regularly reported to the primary care provider. For patients who have recently had:
- Stable angina
- Angioplasty
- Stenting
- Valve replacement
- Coronary Artery Bypass (CABG)
- Myocardial infarction (heart attack)
- Heart transplant
- Congestive heart failure
- Peripheral artery disease
Clinical Fitness Program
In this prevention and maintenance program, patients reduce risk factors for heart disease and nurture long-term lifestyle changes through personalized exercise programs, telemetry and blood pressure monitoring, and aggressive cholesterol and nutrition education.
For patients who are at risk of heart disease, or have:
- Diabetes
- Metabolic syndrome
- High cholesterol
- High blood pressure
- Low exercise tolerance
- Obesity
- COPD
In order to participate in an outpatient cardiac rehabilitation program, you must have approval from your doctor. Cardiac rehab is covered by most insurance programs.
